Palliative Care For our Residents and Patients:
Palliative Care is an approach to care that focuses on the prevention and relief of suffering and on fostering the best possible quality of life for residents/patients and their families, regardless of their particular health situation or the stage of any disease process. The resident/patient and their family members are involved in care planning with the assistance of nursing, therapeutic, social work, and spiritual care staff members. Palliative care focuses on the whole person and as such expands the traditional disease-model approach by focusing on a holistic plan of care that enhances the quality of life for each resident/patient and their family through an integrated approach that addresses each resident/patient’s physical, psychological, emotional and spiritual well-being.
